Employee Onsite Flu Shot Clinics
We will host both Cigna and Kaiser Permanente to campus for Employee Flu Shot Clinics in Community Commons, Room 1800 on Monday, October 28th from 10am - 2pm and Tuesday, October 29th from 11am - 5pm.
Who:听
All 快活app Employees and their spouses/partners may receive a flu shot on campus. Cigna Members and Kaiser Permanente Members will receive a flu shot at no cost, as this is considered preventative care. Please bring your insurance cards. Non-Cigna Members and Non-Kaiser Permanente Members are able to get a flu shot as a cash cost of $10.00. Please bring cash, as cards will not be accepted. Please register online using the link or QR code below听for a time slot to register in advance and to fill out the electronic consent form below. We have one link for Cigna and one link for Kaiser Permanente. Walk-ins are also available onsite with paper consent forms. Following your flu shot, you may go online to review your flu shot confirmation.听

- Records are uploaded into the Life Health portal 5 days following the flu shot events.听
- Records will be uploaded into your MyCigna portal following the event after uploaded into the Life Health portal.听
